Step 1: Contact Us
The first step is easy—call our office! During your call, we’ll collect basic information including:
Your areas of concern
The services you’re interested in
Your schedule restrictions
Insurance information
Please have your insurance details ready. We’ll work with you to understand your coverage.
Step 2: Insurance Verification
Once we have your information, we’ll contact your insurance company to verify your coverage and benefits for therapy services. LEAPS is an in-network provider for:
Blue Cross Blue Shield
Aetna
United Healthcare
Cigna
Tricare
Medicaid (and Medicaid CMOs)
Amerigroup
CareSource
Peach State
Katie Beckett Deeming Waiver
Please contact us if you do not see your insurance listed and we can discuss other funding sources.
Step 3: Prepare Your Documentation
Before your first evaluation, we’ll need the following:
Prescription from your primary care physician, including:
Prescribed service
Diagnostic Report
Any recent evaluations
Any recent evaluations Current evaluation (within the last six months)
After your insurance benefits are confirmed, we will call you to schedule your initial evaluation.
Once the evaluation is submitted and approved by your insurance, Leaps will schedule your first appointment and begin providing the personalized care your child needs
