Getting Started

Getting started with services is easy and our staff is here to help, your first step is to call our office. We will take basic information including your area(s) of concern, the services requested, schedule restrictions, and your insurance information. When you call the office please have your insurance information available. We will gladly contact your insurance company to determine your coverage and benefits for therapy services. Once your insurance benefits are determined, we will call you to schedule the appropriate services. LEAPS is an in-network provider for Blue Cross Blue Shield, Aetna, United Healthcare, Cigna, Humana, Tricare, and Coventry (pending). We will bill all other insurances as an out-of-network provider. We contract with Medicaid including SSI Disability, Katie Beckett Deeming Waiver, and Amerigroup.

Scheduling your first appointment

Prior to your first appointment, we will need the following:

– Prescription from your primary care physician must include: 

Prescribed Service
Diagnosis
Frequency
Duration
– Current evaluation (We will accept outside evaluations completed within six months)
– Current IEP/IFSP required if your child has Medicaid/Amerigroup and receives services through school or Babies Can’t Wait
